About Oncoplasty

“Oncoplasty” is a medical word related to surgical procedures used to remove tumors and to achieve best aesthetic result possible, conservative or reconstructive, on the affected body area. Within oncoplasty procedures, this web site want to show those related to breast cancer, in particular the procedure known as “Wise pattern” (from the United States surgeon Rober J. Wise, first presenting on 1956) and its modification invented by Prof. Santanelli di Pompeo di Pompeo.

As previously mentioned in this web site, when following the clinical investigation from the physicians it is possible to resect the tumor by removing only part of the breast gland, then a quadrantectomy is performed. The word is related to the description of the breast mound as a clock quadrant divided into four equal sections, two superiors and two inferiors. By mean of a quadrantectomy, only one section, the one including the tumor is removed.

Tumors located into inferior quadrants are easy to be resected with this procedure, while until few years ago it was more demanding to resect those located into superior quadrants. It was usually hesitating in the décolleté area a more or less visible hollowness. This problem has been solved by the procedure “modified Wise pattern” described by Prof. Santanelli di Pompeo di Pompeo, achieving results similar to the one obtained with the resections from the inferior quadrants.

Inferior Quadrantectomy according to Wise

This procedure consists of the application of the surgical principles of the reduction mammaplasty technique with the “Wise pattern” to the inferior quadrantectomy operation, useful to reduce large breast.

Scars are absolutely similar to those following a breast reduction mammaplasty, i.e. around the nipple and areola complex, from this down to the inframammary sulcus and along it. Like all scars, these too reduce their visibility during time. With large breast the procedure is usually bilateral to achieve a symmetric result. Patients after weight excursion, aged and pluriparous, might end-up with an unexpected better cosmetic result.

Superior Quadrantectomy according to Santanelli di Pompeo di Pompeo

This technique consists of a modification of the “Wise pattern” procedure described for breast reduction of the inferior pole, to allow for a superior quadrant resection hesitating in a small volume reduction but with a good final aesthetic result.

Final scar line will be positioned along the (medial or lateral) quadrant affected by the tumor, around the nipple and areola complex, vertically down to the inframammary sulcus and along half of it, opposite to the affected side.

Both procedures achieve a result with a natural breast shape, smaller, higher and with scar not visible on the décolleté, which allow to continue wearing dresses and swimming suites with comfort.
